Our vision is for every person with a disability to feel in control, connected, and confident with their NDIS plan. Through open communication, genuine relationships, and expert coordination, we aim to build a future where everyone can thrive.

What is a Support Coordinator & what does a Support Coordinator do?

A Support Coordinator is like a guide who helps you make the most of your NDIS plan. We work alongside you (and your family or carers if you’d like) to make sure your plan is not just a document, but something that truly works for your life.

Here’s what that means in everyday terms:

  • Explaining your plan: NDIS plans can feel confusing, with lots of words and sections. I take the time to go through your plan with you, in plain language, so you understand what supports you can use and how they fit your goals.

  • Finding the right supports: You don’t have to figure it out on your own. I connect you with services and providers that meet your needs, whether that’s therapy, personal care, community activities, or something else. My job is to match you with the right people and organisations.

  • Making things easier: Instead of you having to make lots of phone calls or send endless emails, I can help coordinate appointments, link providers together, and make sure everyone is on the same page. This reduces stress for you and your family.

  • Building your confidence: Over time, I’ll help you learn how to manage more parts of your plan if you want to. The goal is for you to feel more independent and in control of your supports.

  • Problem-solving: If something isn’t working, maybe a provider isn’t the right fit or your needs change, I help you adjust things quickly so your supports continue to work for you.

  • Preparing for reviews: When it’s time for an NDIS plan review, I can help gather reports, check progress, and make sure you’re ready to show the NDIS what you need going forward.

If you have an NDIS plan

Guiding Hands Support Coordination are here to hold your hand through the journey.

As a Level 2 Support Coordinator, our role is to make your NDIS journey smoother and less stressful. We can help you find and set up the right supports and therapists, and we’ll organise the service agreements so everything is clear from the start. Once you’re linked in, we’ll stay in touch with your therapists, keep an eye on how things are going, and bring all the reports together when it’s time for your plan review.

Managing your NDIS budget can feel overwhelming, but we’ll work alongside you to keep track of your funding and make sure you’re getting the most value from your plan. If any complex issues come up, we’ll be there to guide you through them, explain what they mean, and help you make confident decisions.

When your plan is due for renewal, we’ll support you through the whole process. This includes helping you reflect on your goals, prepare for what’s next, and even being present on the review call to back you up with the more detailed questions.

In short, we’re here to walk beside you every step of the way, making sure you feel supported, organised, and confident as you work towards your goals.
